He attended Franklin College Ohio, graduated in medicine from the University of Louisville Kentucky, in 1895 and was commissioner of health of Mingo County, West Virginia, (1895-1900). He also was surgeon for the Norfolk and Western Railway, (1895-1913) and surgeon in chief of West Virginia State Hospital, (1899-1913). Entering politics, he was commissioner of district roads of McDonwell County, (1900-05), a member of the McDowell County Court, (1906-12) and a member of the West Virginia State Senate, (1908-12). Hatfield was a lifelong Republican. In 1912, he won election as governor of West Virginia; he served from 1913 until 1917. After leaving office, he served in World War I as a Major in the U.S. Army Medical Corps, (1917-19) and after the war, was appointed a Lieutenant Colonel in the Medical Officers Reserve Corps. In 1928, he was elected as a Republican to the U.S. Senate, serving until 1935. An unsuccessful candidate for reelection, he resumed the practice of medicine until his death.
